- Secure, coordinate, and review medical records, advance directives, and key healthcare information
- Oversee medication management by reconciling frequent changes across sites of care during transitions, filling prescriptions and medication cassettes, and monitoring compliance
- Assist in navigating electronic medical records and patient portals to ensure timely access to information and effective communication with providers
- Coordinate and confirm provider appointments, prepare agendas, and accompany clients to visits for added support and advocacy
